Our Commitment to Your Privacy
NORRENNE, PLLC is committed to protecting the privacy of your health information. We are required by law to maintain the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. This Notice applies to all records of your care generated by NORRENNE, PLLC.
How We May Use and Disclose Your Health Information
Treatment: We may use and disclose your health information to provide, coordinate, or manage your healthcare and related services. For example, your provider may share information with a specialist or collaborating physician as part of your care.
Payment: We may use and disclose your health information to obtain payment for services we provide to you. Because NORRENNE, PLLC is a self-pay practice, we do not bill insurance. Payment-related disclosures are limited to your direct financial transactions with our practice.
Healthcare Operations: We may use and disclose your health information for our internal operations, including quality assessment, training, compliance activities, and administrative functions necessary to operate the practice.
Appointment Reminders: We may contact you to remind you of upcoming appointments via phone, text, or email using contact information you have provided.
As Required by Law: We will disclose your health information when required to do so by federal, state, or local law, including reporting requirements.
Public Health Activities: We may disclose your health information to public health authorities for activities such as disease reporting, product recalls, and FDA safety surveillance as required by law.
Serious Threat to Health or Safety: We may use or disclose your health information if we believe it is necessary to prevent a serious and imminent threat to your health or safety or the health or safety of another person.
Uses and Disclosures Requiring Your Authorization
The following uses and disclosures of your health information will be made only with your written authorization:
Most uses and disclosures of psychotherapy notes (where applicable), uses and disclosures of your health information for marketing purposes, and the sale of your health information.
You may revoke any authorization you provide to us at any time by submitting a written request to hello@norrenne.com. Revocation will not apply to information already used or disclosed in reliance on your authorization.
Your Rights Regarding Your Health Information
Right to Access: You have the right to inspect and obtain a copy of your health information that we maintain. Requests must be submitted in writing to hello@norrenne.com. We may charge a reasonable fee for copies.
Right to Request Amendment: You have the right to request that we amend your health information if you believe it is incorrect or incomplete. We will respond to your request within 60 days.
Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we have made of your health information. This right applies to disclosures made for purposes other than treatment, payment, or healthcare operations.
Right to Request Restrictions: You have the right to request restrictions on how we use or disclose your health information. We are not required to agree to your request, but if we do, we will comply with the agreed restriction.
Right to Request Confidential Communications: You have the right to request that we communicate with you about your health matters in a certain way or at a certain location. We will accommodate reasonable requests.
Right to a Paper Copy of This Notice: You have the right to a paper copy of this Notice at any time upon request.
Right to Notification of a Breach: If a breach of your unsecured protected health information occurs, we will notify you as required by law.
Texas-Specific Rights — Texas Medical Privacy Act (HB 300)
In addition to your federal HIPAA rights, Texas law provides additional privacy protections. NORRENNE, PLLC complies with the Texas Health & Safety Code Chapter 181. We will not use your health information for marketing purposes without your specific written consent. All workforce members who handle PHI receive training on Texas medical privacy requirements.
Changes to This Notice
We reserve the right to change this Notice and to make the new Notice effective for all health information we maintain. The current Notice will be posted in our office and on our website at www.norrenne.com. You may request a copy of any revised Notice at any time.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with NORRENNE, PLLC or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized for filing a complaint.

These policies exist to protect your appointment, respect the time of every patient on our schedule, and allow us to deliver the unhurried, personalized care that defines NORRENNE.
We understand that life happens. We ask only that you communicate with us as early as possible when your plans change.
Appointment Deposits
A deposit is required to book and hold all appointments at NORRENNE, PLLC.
Deposits are $100 and applied as a credit toward your total on the day of your service. If you reschedule with adequate notice, your deposit transfers to the new appointment. Deposits are non-transferable to another person.
Forfeited for no-shows or cancellations with less than 24 hours notice.
Sexual health procedures (O-Shot, P-Shot, Clitoxin, Scrotox, Wing Lift): $300. Applied toward your treatment total. Forfeited for no-shows or cancellations with less than 24 hours notice.
Hormone and wellness consultations: $100. Applied toward your treatment total. Forfeited for no-shows or cancellations with less than 24 hours notice. Labs & paperwork must be completed 24 hours prior to appointment.
Package and membership appointments: Per your Package Agreement terms. Late cancellations and no-shows result in $100 charge to your account.

Cancellation Policy
We require cancellation notice by the following deadlines. Notice must be given by phone or text to 281-508-2117. Email alone is not sufficient for same-day or short-notice cancellations.
Consultations: 24 hours advance notice required. Late cancellations result in forfeiture of deposit.
All treatment appointments: 24 hours advance notice required. Late cancellations result in forfeiture of deposit.
Rescheduling with adequate notice: Your deposit transfers to the new appointment at no charge. Rescheduling after the cancellation deadline is treated as a late cancellation and your deposit is forfeited. A new deposit is required to rebook.
Rescheduling limit: The same appointment may be rescheduled a maximum of two times. A third reschedule requires a new deposit and is at management's discretion.
Emergencies and illness: Genuine medical emergencies and acute illness are handled with compassion at management's discretion. Please contact us as early as possible. Documentation may be requested for deposit transfer consideration.

Late Arrival Policy
Please arrive 5 minutes before your scheduled appointment if all of your paperwork is completed online & 10 minutes early if you need to allow time for any necessary paperwork and preparation.
Arrivals more than 10 minutes late: Your appointment may be shortened to protect the next patient's appointment time. The full service fee applies regardless of shortened duration.
Arrivals more than 15 minutes late: Your appointment may need to be rescheduled and treated as a late cancellation. Your deposit may be forfeited. We will always do our best to accommodate late arrivals when our schedule permits, but this cannot be guaranteed.

No-Show Policy
A no-show is defined as failing to appear for a scheduled appointment without prior notice.
No-shows result in automatic forfeiture of your deposit. A no-show fee of up to the full scheduled service price may be charged to the card on file.
After one no-show, a larger deposit may be required for future bookings at management's discretion.
After two no-shows, NORRENNE, PLLC reserves the right to require prepayment in full before scheduling future appointments, or to decline future bookings.
Package clients: A no-show appointment is deducted from your package at the full individual retail price of that service, per your Membership and Package Agreement.

On rare occasions, NORRENNE may need to reschedule or cancel an appointment, for example due to illness, an emergency, or circumstances beyond our control. If this happens, we will contact you as soon as possible to find a new time that works for you.
If NORRENNE cancels or reschedules your appointment, your deposit is fully protected and will be applied to your rescheduled visit, or refunded if you prefer.
We understand your time is valuable, and we hold ourselves to the same respect for your schedule that we ask of you.

A valid credit or debit card must be on file to book and maintain any appointment at NORRENNE, PLLC. By providing your card and signing our Financial Agreement, you authorize NORRENNE, PLLC to charge the card on file for appointment deposits, cancellation fees, no-show fees, and balance due at time of service. Your card information is stored securely in our HIPAA-compliant, PCI-DSS-compliant payment system.
